During the recent International Forum on Quality and Safety in Healthcare, I facilitated a specialty poster focus group themed on record keeping. It was thought-provoking to hear different views on the latest ways to combat the everyday issues of ineffective record keeping.

Throughout the tour, the delegates heard a range of ideas, from the small and simple, to the more ambitious and innovative. The overarching message from each passionate presenter was the same, though; we need to make things work better for the benefit of the patient. What was clear is that this issue is still problematic, and not specific to one profession alone.

We ask a lot of questions through medical school. History taking and skills practice bring up some valid questions, but are we always asking the right ones?

The thought of visiting hospital can be a scary and unpleasant experience for many patients, so after asking all the routine questions; why not ask: “What can I do to improve your stay?”

The Ask One Question campaign is doing just that. It’s a world-wide, student-led healthcare improvement initiative, encouraging students to ask their patients: “What can I do to improve your stay?” and, when possible, act on their requests. We know from international learning the value and importance of developing a common improvement language. This helps us to communicate within NHS Wales about how we can make real, worthwhile and lasting change happen.

And that’s the point of Improving Quality Together: making worthwhile changes. Often, changing the little niggles people have every day, which they feel is stopping them doing their job properly, can make a huge difference. It can free up staff time, and make the system we work in a lot more effective and efficient.

But we also need to know that the change we’re making is a worthwhile one, so Improving Quality Together helps us to think about how we test these ideas for change on a small scale before going bigger. Continue reading »

Here’s a challenge. How do we best make sure patients get person-centred care at a time when resources in the whole of the public sector are being stretched? Part of the answer lies in co-production, something that is currently being widely talked about in Wales.

 If a part of the healthcare system is outdated, cluttered, or simply not providing person-centred care, among the first to notice this will be patients. When we’re working within an already established system, it can sometimes be hard to see how things could be done differently.
